California Lifeline Cellphone Sign Up through Assurance Wireless

Come prepare and provide a document from each group A, B and C to qualify for the program.

(A) Proof of Benefit document
• Medi-Cal cards
• EBT cards (food stamps/FSNAP)
• Suplemental Security Income (SSI) award letters
• Or statements of benefits for any of these programs (WIC, TANF, CALWORKS, WTW)

(B&C) Documents that validate both Address and Identity
• Unexpired Driver’s License
• Income Document (Includes: W2, Prior year’s State, Federal or Tribal tax return)
• Government Assistance Program
• Unexpired U.S. government, Military, State, or Tribal issued ID

(B) Documents that validate Identity only
• Unexpired Passport
• Permanent Resident Card
• Certificate of U.S. Citizenship
• Certificate of Naturalization or Citizenship
• Unexpired Matricula Consular Card (Customer Must Have a Valid Social Security)

(C) Documents that validate Address only
• Utility Bill (dated within the last 365 days)
• Current Mortgage or Lease Statement
• Statement of Benefits
• Current Registration Document from the California DMV
